Jasmine L. Moore, age 20 passed away Thursday, August 29, 2024, at Ochsner Rush Health.
Survivors include her parents, Billy and Jessica Moore; siblings, Joey Frazier, Mason Lambert, Nicole Heitz (Tyler), Charles Moore, and Brittany Taylor; nephew, Charlie Taylor; niece, Heidi Heitz; grandmother, Kay Brooks; great-grandmother, Mrs. Frazier Phillips; numerous extended family and friends.
She was preceded in death by her grandfather, Bobby Joe Lambert, great-grandfather, Mr. Frazier Phillips.
